Among the infants without pneumonia, bacteria were cultured from the lungs of 36% of stillborn infants and infants who died within the first 24 hours, 53% of infants who died between 24 hours and 7 days of age, and 75% of infants who died between 7 and 28 days of age. Maden and associates165 identified congenital pneumonia (defined by the presence of neutrophils in the alveolar spaces) in 45% of neonatal autopsy cases; isolated organisms included S. Barson166 identified bacteria in lung cultures at autopsy of 252 infants dying with bronchopneumonia; positive cultures were obtained in 60% of infants dying on the first day of life and in 78% of infants dying between 8 and 28 days of age. Bacteria were cultured at autopsy from the lungs of many infants with and without pneumonia. Information about bacterial etiology of pneumonia also can be obtained by culturing blood, tracheal aspirates, and pleural fluid and by needle aspiration of the lungs of living children with pneumonia. The bacterial species responsible for fetal and neonatal pneumonia are those present in the maternal birth canal; included in this flora are gram-positive cocci such as group A, group B, and group F167 streptococci and gram-negative enteric bacilli, predominantly E.

Results are expressed as an avidity index, that is, the percentage of antibodies that resist elution. Toxoplasma antigens are first separated by electrophoresis, according to their molecular weight, before being transferred to a nitrocellulose membrane. Serum is incubated with this membrane, and antibodies are revealed by an enzymatic tracer. The technique has high sensitivity but is not automated; interpretation is sometimes difficult. A large number of kits are commercially available that are very useful for routine frontline screening tests. However, they vary in characteristics and quality and have certain limitations in terms of performance; it is also sometimes necessary to send the serum samples to a reference laboratory, where more highly specialized tests are available to confirm atypical findings and to help establish the timing of an infection (Table 31-4). Biologists and clinicians should also be aware of another limitation, which is the impossibility of comparing the findings obtained with different kits or performed by different laboratories because of large intertest variations in the principles and mixtures of antigens used, which influence the kinetics and performance. Upon reaching a peak after 2 or 3 months, IgG titers stabilize at a plateau phase for several months before declining progressively and can decrease to very low levels with time but persist throughout life. This persistence of IgG provides a reliable means to recognize which pregnant women or women of childbearing age are immune because of a past infection and who are seronegative and therefore susceptible to infection. A positive test for IgG before pregnancy indicates that the fetus is not at risk, with the possible caveat that rare cases of congenital toxoplasmosis have been reported in children born to mothers who were immune before conception; these rare cases involved specific medical conditions in the mother that were likely to weaken her immunity and thereby to foster recrudescent infection or reinfection with a different strain. The difficulty of interpreting positive IgM tests in the presence of IgG antibodies is even greater when the IgM tests used only provide qualitative results and cannot distinguish between low-to-moderate levels versus high levels that are more likely to indicate a recent infection. False-positive test results for IgM and the possibility for natural IgM (see "Natural Immunoglobulin M") represent a further challenge for the interpretation of test results.
